We look forward to serving you! In preparation of your first visit, please do the following:
- Plan to arrive 15-20 minutes prior to your scheduled appointment to complete the registration process
- Contact primary care physician to request your medical records be sent to us
- Bring insurance cards and a photo ID
- Bring all medication bottles with you to the visit
- Depending on your insurance plan, be prepared to make your co-payment during the registration process
- If you are uninsured, be prepared to pay a deposit towards your services
- We accept cash, personal checks and credit cards (Mastercard, Visa & American Express)
- If your insurance company requires a referral, please obtain this prior to your scheduled appointment
- New patients are encouraged to contact their current insurance carrier to verify our physicians are currently participating with their specific insurance plan.
- Please call if you are unable to keep your scheduled appointment. We request 24 hour notification. We will be glad to reschedule a time that works for you.
